- Kettering Health Medical Group, formerly known as Kettering Physician Network, employs more than 700 providers, including physicians and advanced practice providers, throughout the Greater-Dayton and Cincinnati areas.

Responsibilities & Requirements
Essential Responsibilities
• Meets population specific and all other competencies according to department requirements.
• Promotes a Culture of Safety by adhering to policy, procedures and plans that are in place to prevent
workplace injury, violence, or adverse outcome to associates and patients.
• Work with health plan partners to address preventive and chronic care needs of shared members.
• Maintains expert knowledge of core measures and other pertinent quality initiatives
• Maintain technical competency and remain current in technology and changes in the industry.
• Completes chart reviews for quality improvement initiatives including gaps in care as well as identification of
HCCs; Communicates gaps & HCC information to provider and/or practice staff.
• Other reviews/projects as assigned
• Meets deadlines and timeframes for reviews and reporting
• Maintains strict confidentiality of all data and information
• Participates in conference calls and meetings to maintain knowledge and understanding of quality initiatives
• May assist with the coordination and management of event reports from identification through closure
• May provide outreach to patients in need of preventive and chronic care services through electronic and
phone communication, including letters, text messages and/or phone calls.
• May help patients address barriers to care, such as transportation, income, insurance, through education,
advocacy and/or referral.
Other Job Responsibilities
• Participates in activities which own and promote professional growth & development, as well as seeks out
opportunities for organizational growth of key stakeholders.
• Conducts educational programs regarding quality improvement to practices, as well as other internal and
external customers.
• Participates as a member of quality-related committees and/or workgroups and is assigned administrative
support responsibilities as needed including necessary documentation.
• Other duties/projects as assigned by the Network Manager Ambulatory Quality
• Responsible for compliance with Organizational Integrity through raising questions and promptly reporting
actual or potential wrongdoing.
• All other duties as assigned.
