Responsibilities & Requirements
The role of the Care Coordinator is to support the interdisciplinary medical team in provision of care through coordination of activities across and within healthcare delivery sites. The Care Coordinator will facilitate the interface between the medical home team and regional support teams in caring for patients with complex health needs. The assistance of the team will be pivotal in creating access, coordination, and interface between providers and patients as the needs of their population rises.
The Care Coordinator will provide direct and indirect assistance to the care manger RN, social worker, pharmacy, physicians and advanced practice providers. The Care Coordinator will perform duties to support the specific population(s) identified including, but not limited to: patient outreach, chart review, coordination of services, care plan support, running data and dashboard analysis, and communication to team members.
Key improvements the Care Coordinator will focus on include: improved access, optimized utilization management, proactive panel management, quality outcomes of disease management, and excellent customer experience.
Upholds the standards of the system-wide customer service program. Performs duties for practice or Network commensurate with applicable certifications and licensure when deemed necessary by leadership. Case load will be determined by accepted Regional and Network standards.
Performs other duties as assigned.
Education: Graduate of accreditaed LPN Program and an active Ohio Licensed Practical Nurse (LPN)
